New Health Partnerships provides resources to patients, members of healthcare provider teams and family members, as well as to the general public, at no or low cost, to help them take effective action to improve health care, and most especially in the area of collaborative self-management within the context of chronic care.

NewHealthPartnerships.org is owned and operated by the Institute for Healthcare Improvement, a Michigan nonprofit corporation. By using our website, you agree to the following Terms of Use, which constitute an agreement between the Institute for Healthcare Improvement and you. If you do not agree to these Terms of Use, do not use the NewHealthPartnerships.orgwebsite. When we refer to ourselves as "we," "us," "our," and similar terms we mean those who are involved in the development, management and maintenance of the NewHealthPartnerships.org site, and more generally, the Institute for Healthcare Improvement, including any subsidiary organizations and/or projects we control. The Institute for Healthcare Improvement controls newhealthpartnerships.org from its offices within the Commonwealth of Massachusetts in the United States of America. newhealthpartnerships.org can be accessed from any of the United States and from other countries worldwide. Since the laws of each state or country able to access newhealthpartnerships.org may differ, by accessing IHI.org both you, New Health Partnerships and the Institute for Healthcare Improvement agree that the statutes and laws of the Commonwealth of Massachusetts will apply to all matters relating to use of newhealthpartnerships.org. Both you, New Health Partnerships and the Institute for Healthcare Improvement submit to jurisdiction in Massachusetts and further agree that any cause of action arising under these Terms of Use shall be brought exclusively in any state or federal court located in Boston, Massachusetts.

New Health Partnerships and its parent organization The Institute for Healthcare Improvement makes no representation that materials on newhealthpartnerships.org are appropriate or available for use in other locations, and accessing them from territories where their contents are illegal is prohibited.
